Why you wake at 3am in perimenopause, and what low progesterone has to do with it

So many women I see in clinic can tell me the exact time they wake. Ten past three. Half three. They wake with a heart rate that resembles someone being chased, and have absoloutely no idea of the cause.


It's frightening, not knowing what's happening to you, so you go to your GP, you have your bloods taken, and everything comes back normal. You leave with the strong impression that you are imagining things, but something still doesn't sit right.


Let me help you understand. Progesterone is on its way out, and this is why you wake at 3am in perimenopause.


Perimenopause is usually described as an oestrogen decline, because the symptoms everyone recognises, the flushes and the night sweats, are largely low oestrogen symptoms. But the first hormone to change is generally progesterone, and it starts changing years before periods do, as early as 35!


Progesterone is only made in meaningful amounts after ovulation. The follicle that released the egg becomes a temporary gland, and that gland makes progesterone for the second half of the cycle. No ovulation in a given month means very little progesterone in that month, even if you still bleed on time.


From the late thirties onwards, ovulation becomes less reliable. Some cycles you ovulate well. Some you ovulate weakly. Some you do not ovulate at all, and still have a bleed. So progesterone does not decline in a tidy line. It becomes unpredictable, which is part of why perimenopause feels so destabilising. One month you feel like yourself, and the next you do not recognise your own reactions.


Why this shows up as anxiety and 3am waking


Progesterone is not only a reproductive hormone. It is the precursor to allopregnanolone, a neurosteroid that acts on GABA receptors in the brain. GABA is your main calming signal. It is what allows you to put the day down, fall asleep, and stay asleep through the night when cortisol naturally begins to rise - this is what helps you get out of bed in the morning.


When progesterone is low, allopregnanolone is low, and that overnight cortisol rise kicks in easier. You wake up, and your body treats this normal biological event as an alarm. Then you lie there, and the anxiety generated by lying there awake adds to it. The same biochemical mechanism explains the irritability, and often rage. Less buffer between you and a stressful day means an ordinary day starts to feel like a demanding one.


There is a second layer to this, and it is where my nursing background matters to me. The stress (HPA) axis and the reproductive (HPO) axis are in constant conversation. When the body is under sustained pressure, whether that is a job, small children, undereating, over-exercising, poor sleep or unprocessed grief, it deprioritises ovulation. Less ovulation means less progesterone, which means less capacity to handle stress, which means more pressure. That loop is very common, and it is breakable once you can see it.


Why your blood test looked fine


Progesterone was often not tested at all. When it was, it may have been tested at the wrong point in the cycle, and progesterone varies enormously across a single month. A test in the first half of the cycle will look low in every woman, and a test taken at a random point in a woman with irregular cycles is very hard to interpret.


Reference ranges are also wide by design. They are built to catch disease, not to describe the difference between the way you function now and the way you functioned at thirty two. This is not an attack on general practice. Acute medicine saves lives, and a ten minute appointment is not the setting for a deep conversation about your cycle and where lifestyle might be playing a part.. It is simply a gap, and it is the gap I work in.


What actually helps


Progesterone responds to ovulation, and ovulation responds to safety. That is the honest answer, and it is less marketable than a supplement.


Eating enough matters more than most women want to hear, particularly women who have spent two decades being praised for being 'skinny' or 'disciplined'. Chronic under-fuelling is one of the fastest ways to turn off a good ovulation. Blood sugar stability matters for the same reason, which in practice means protein at breakfast rather than coffee on an empty stomach, and meals built from whole foods with protein, fat, fibre and carbohydrate together rather than alone.


Thyroid function and iron levels are worth investigating properly rather than glancing at one marker, because both influence ovulation directly and both are commonly under-assessed in women who are told they are simply tired.


Stress load has to come down somewhere realistic. Not in theory, and not by adding a fifth thing to your morning or evening routine. Something has to actually be removed.


And the movement question deserves more honesty than it usually gets. Cold water, hard training, evening cardio, slow yin yoga, none of these is universally good. Evening cardio can lift cortisol at exactly the wrong hour for a woman who cannot sleep. Cold exposure can be too much stimulus for a nervous system that is already on edge. Slowing down can feel deeply unsafe for a woman in functional freeze, which is why being told to rest sometimes makes things worse. What helps depends entirely on where your nervous system currently sits, and that is worth working out before you commit to a protocol somebody else loved.


Where testing helps, it helps because it takes the guessing away. A properly timed progesterone test, or a DUTCH test that looks at what your body is actually producing and how it is metabolising it, turns a vague sense of 'something is wrong' into information you can actually work with.


If you take one thing from this, take this. The symptoms are real, they are physiological, and they have a biochemical mechanism. When you try to solve them without the relevant information, you look for a needle in a haystack.
